We are seeking an inspirational, caring, and motivated person who believes they can make a difference in the lives of the young people that we support, to join our Hub Team as a care coordinator for our Supported Living Service.

· You will be supporting young people in their home and in the community, taking them on social activities, helping them to pursue their interests, and develop independent living skills. No two days will be the same!
· As Care Coordinator, you will be working with our young adults to enable them to lead their best lives and be the best version of themselves. You will also assist the management team in maintaining a safe and enabling environment that provides positive experiences to young people as well as being a role model to the staff team and imbedding Progress values within daily practice.
· You will have a small amount of admin time off Rota and the rest will be direct support. The role will require you to work unsociable hours, including early mornings, evenings, weekends, and bank holidays and be part of a management out of hours on call Rota. You will also have competent IT skills and ideally have access to your own transport.
· You will be based in one of our supported living services.
· The hours for this position are full-time, 38 Hours. You will need to be fully flexible, including evenings, weekends, and Bank Holidays
